Through assessment, planning and tailored support, our Wellness approach ensures that you’re actively involved in the process. Whether assistance is needed for a short time, changes in intensity over time, or is ongoing, our focus remains on helping you take steps toward achieving your goals.
Our team will work alongside you to identify your abilities and create opportunities to enhance your independence and wellbeing.
Eric Hart Allied Health Services are funded by the Commonwealth Government of Australia through the Commonwealth Home Support Programme (CHSP). These services are designed to restore, improve, or maintain the health, wellbeing, and independence of older adults aged 65 and over who are registered with My Aged Care.
If you are aged 65 or over you can also access our allied health services directly through fee-for-service, or the following government-funded programs:
- Home Care Package (HCP) funding
- NDIS funding
- The Department of Veterans’ Affairs (DVA)
Our services are conveniently located in Lara, Victoria, at:
• Eric Hart Allied Health Centre
• St Laurence Park Retirement Village
We also welcome you to visit Jubilee Park, our senior outdoor exercise area in Lara, designed to enhance balance, strength, and mobility.
- If you are accessing this service through CHSP: simply contact My Aged Care for a referral first. Visit the My Aged Care page or call on 1800 200 422.
- If you are accessing this service through a Home Care Package (HCP) talk to your Case Manager and they can provide a referral. The programs you choose will be costed to your Home Care Package.
- If you are accessing this service through DVA referrals need to include a doctor or hospital referral.
- You can access our services directly through fee for service, simply contact us on 1300 558 368 to enquire.
